Claim Procedure

Step 1.

Report an accident & collect necessary document

  • Medical Expenses & Sequela Benefit Claim : Following the incident, you may send a report to appointed company prior to get a treatment at the nearest hospital/clinic.
  • Death Benefit Claim : Following death incident, you may send a report to appointed company.
  • Please ensure to collect all the necessary documents.
Step 2.

Notify us & submit document

  • You may notify us of your accident and ask for a claim note by contacting our phone at 021 - 252 3110 ext. 288/373 or Fax at 021 - 252 4083 during working hour (Mon-Fri, 08:00 -17:00 WIB).
  • We will send a claim note directly to your email
  • You should fill out the claim note and complete all required documents before submit it to us.
Step 3.

Assesment refer to policy condition

  • If your claim is covered by your insurance policy, we will arrange complete full claim payment.
  • However, If your claim is not covered by your insurance policy, we will propose an adjustment which you need to sign to proceed with the claim process.
  • If your claim is rejected, we will issue a repudiation letter to you
