1. Original Claim Note (duly completed and signed) and do not forget to write the detailed chronology and diagnosis.
2. Original Medical Invoices
3. Medical Record from the attending doctor ( in case that medical expense is exceeding IDR 5,000,000.00).
4. Statement letter from Company informing employment matter of the insured.
5. Copy of Employee ID Card
6. Police report and copy of driving license. ( In case related with traffic accident).

2. Death Statement Letter from Company.
3. Death Statement Letter from Local Goverment.

5. Copy of ID Card / KTP of Legal Heir.
6. Accident Report from Company (detailed of accident's Chronology.
7. Accident Police report and driving license 9in the case that the accident related with traffic accident).
8. Coroner's Report from Hospital (Visum et Repertum).
9. Copy of Kartu Keluarga.
10. Copy of ID Card / KTP of the insured person.
11. Statement Letter from Company Informing to whom the death benefit would like to be transferred (including Bank Account Information).

7. Photograph part of the body which has permanent disablement.
